먼저 저희 e Pharmacy 를 처음으로 이용하시는 분은 조제 신청시 신규환자 등록카드를 반드시 작성해 주세요. 이미 이용하시던 분들은 조제 신청만 하시면 됩니다.

“신규환자 등록 카드”

환자분 영문이름, 한국이름, Healthcare number, 주소, 전화번호 (cell# 필수), 약물 알러지, 사설 보험 (private drug insurance) 유/무, care giver 이름 + 전화번호 (해당하는 경우)

“조제 신청서”

영문이름, 전화번호, 약 이름, 수량(day supply), 리필 y/n, 기타 특이사항

1. 새로운 처방전을 받아야 할 경우: 닥터 오피스에 저희 약국 fax 번호를 주시고 처방전을 보내달라고 요청하시고 (Fax 604-930-9785), e Pharmacy 웹사이트에 조제 신청을 꼭 남겨 주시기 바랍니다.
2. 처방전을 받아서 가지고 계신  경우: e Pharmacy는 on-line 처방전을 다루기 때문에 닥터 오피스에서 직접 fax 로 보낸 처방전만 유효합니다. 만약 이미 처방전을 종이로 받으셨다면, 닥터 오피스에 사정을 말씀 하시고 아래의 fax 번호로 처방전을 다시 보내 달라고 요청해 주십시오. (Fax 604-930-9785) (본인이 이메일로 보내신 처방전은 유효하지 않습니다.)
3. 다른 약국에 처방전 리필이 남아 있으신 경우: 다음의 양식에 따라 신청해 주시면 저희가 해당 약국에서 처방전을 transfer 해 옵니다.
